Elaborarea acestui manual este un eveniment editorial remarcabil. În general, conceptia, organizarea si publicarea unei serii de 10 volume ce acoperã toate specialitãtile chirurgicale si subspecialitãtile constituie o activitate dificilã, care implicã un volum de muncã important si nu este, în mod evident, usor de realizat, deoarece cere o echipã de autori care nu numai cã sunt hotãrâti sã îndeplineascã aceastã activitate, dar au si formatia necesarã pentru a transmite un mesaj stiintific clar si eficient.
Sunt onorat de solicitarea colectivului de redactie al prestigioasei reviste "Chirurgia" de a prezenta volumul de urologie. Trebuie sã spun cã am avut câteva avantaje din acest punct de vedere. În primul rând, asemãnãrile între limbile românã si italianã mi-au fãcut sarcina mai usoarã.
Faptul cã sunt co-autor la câteva capitole ale acestui volum mi-a permis sã am o imagine clarã asupra calitãtii, bazei stiintifice si nu în ultimul rând originalitãtii volumului de urologie ca parte a acestui tratat.
Ideea asupra cãreia cele 15 capitole ale cãrtii sunt fundamentale este aceea a eficientei informatiei si nu a unui ansamblu enciclopedic. Si aceasta deoarece principalul obiectiv al acestui tratat este reprezentat de cãtre tineri în formare în specialitãtile chirurgicale, care pregãtesc examenul de absolvire.
Pot sã afirm ca în acest extrem de precis volum de autoritate, autorii au îndeplinit câteva probleme prioritare, sub atenta, eficientã si profesionala conducere a prof. Lucan:
· au evidentiat clar importanta examenului clinic si a istoricului cazului, ceea ce faciliteazã un diagnostic corect, rapid si nu în ultimul rând raportul cost-eficientã;
· ilustratiile din acest volum apartin arhivelor informationale ale Institutului Clinic de Urologie si Transplant Renal din Cluj Napoca si atestã uriasa muncã depusã de cãtre echipa acestei institutii si varietatea impresionantã de cazuri. Aceasta se întâmplã când institutia în cauzã are o adresabilitate remarcabilã, ceea ce demonstreazã calitatea asistentei medicale;
· clara modalitate de abordare, structurare si prezentare a subiectelor specifice faciliteazã întelegerea rapidã a subiectului si este idealã pentru aceia care, fie cã se formeazã pentru aceastã specialitate fie au nevoie de o informatie rapidã, dar nu au timpul sau dispozitia de a recurge la manuale enciclopedice.
Rezumând, ideea de bazã care este confirmatã de cãtre cuvântul introductiv al editorului este aceea de eficientã de la prima pânã la ultima paginã.
Cu sigurantã pentru aceia care vor citi aceastã carte, textul concis poate trezi note de întrebare (trebuie sã admitem cã prezentarea esentialului într-o manierã concisã si clarã este de fapt mult mai dificil decât dezvoltarea aceluiasi subiect într-un spatiu editorial întins).
Lista cu bibliografia este mare si adaptatã posibilitãtii de a suplimenta informatia furnizatã de cãtre acest volum, aceste referinte fiind foarte actualizate.
Din cele 15 capitole ale cãrtii am fost în special interesat de capitolul Semiologia Aparatului Urinar, Infectiile Specifice Tractului Urinar si Laparoscopia în urologie. Am fost interesat de acestea deoarece mi-au pãrut cele mai desãvârsite din toate capitolele.
În mod evident, capitolul care abordeazã insuficienta renalã cronicã concentreazã si prezintã o experientã specificã impresionantã în transplant renal, aceea a unui centru care poate fi considerat unul din cele mai eficiente centre de transplant din Europa de est.
Sunt multe tratate de urologie abordabile. Unele dintre ele au o calitate remarcabilã datoritã atât calitãtii autorilor cât si experientei în tratarea si analiza subiectelor specifice.
Majoritatea acestora, mai ales cele mai recente, contin foarte multe capitole cu investigatii paraclinice care utilizeazã cele mai moderne sisteme de diagnosticare. Adevãrul este cã sistemul urinar, prin specificul design-ului sau anatomic, poate fi extrem de riguros de diagnosticat. Totusi, foarte putine din aceste tratate (cu exceptia tratatului Campbell) subliniazã un factor extrem de important: diagosticul clinic si stabilirea istoricului cazului vor fi întotdeauna baza oricãrui diagnostic în urologie.
Urologia este o specialitate importantã, care este diferitã fatã de celelalte specialitãti chirurgicale. Aceastã declaratie se bazeazã nu numai pe faptul cã nu existã personalitate importantã în lume care sã nu fi avut, nu are sau nu va avea nevoie de sfatul unui urolog. Ea se referã si la locul pe care îl ocupã urologia în termeni de incidenta a bolilor urologice la nivel general.
Luând în consideratie aceasta, numãrul specialistilor urologi ar trebui reînnoit în mod continuu cu oameni tineri seriosi, cu entuziasm, capabili de a purta mai departe mesajul stiintific de la predecesori. Unul din elementele fundamentale de care are nevoie aceastã generatie tânãrã pentru a veni cât mai aproape posibil de realizãrile predecesorilor lor este de a fi capabilã de a primi informatia necesarã într-un mod clar, corect si atractiv. Aceste cerinte sunt larg întâlnite în volumul publicat de Prof. Dr. M. Lucan.
Orice tratat medical care rezultã din contributia mai multor autori reprezintã un pericol potential nu numai pentru unitatea stiintificã a cãrtii, dar si pentru unitatea de colaborare între diversi autori si a modului în care subiectele sunt abordate pânã la finalizarea lor.
În sfârsit, editorul cãrtii trebuie sã uniformizeze subiectele scrise de cãtre fiecare autor pentru a mentine ideea unui mesaj stiintific dens, ceea ce s-a dorit de la început.
Existã totusi un sentiment evident care a fost extrem de bine exprimat de cãtre Sir Dn. Innes Williams: "Majoritatea autorilor capitolelor ar fi mult mai fericiti sã scrie aceste capitole pentru cãrtile proprii decât pentru un alt editor".
Nu este si cazul acestei cãrti. Desi acest volum este semnat de multi autori la scara internationalã, uniformitatea capitolelor, care toate exprimã conciziune si eficienta, este vãzutã clar, fãrã a altera personalitatea specificã a celor 16 autori din toate centrele Europene.
Esenta atâtor multe noi concepte într-un singur volum de urologie demonstreazã o maturitate editorialã înaltã. Erorile sau omisiunile care pot surveni frecvent sunt remarcabil diminuate în acest volum.
Pentru a încheia, Benjamin Jowete, un profesor de literaturã de la Oxford, a spus: "Dacã poti condensa o carte mare într-una micã si a pãstra întelesul original si informatia, nu vei fi trãit în zadar".

The elaboration of this textbook is an outstanding editorial event. In general, the conception, the organization and the publication of a 10 volume series covering all surgical specialties and subspecialties is a difficult activity which involves a very important amount of work and is obviously not easy to perform, as it requires a team of authors who are not only determined to undertake this activity, but who also have the necessary training in order to convey a clear and effective scientific message.
I am honored by the request of the editorial board of the prestigious journal “Chirurgia” to review the urology volume. I must say that I had a number of advantages from this point of view. In the first place, the similarities between the Romanian and Italian languages made my task easier.
The fact that I am the co-author of several chapters of this volume allowed me to have a clear picture of the quality, the scientific foundation and not last, of the originality of the urology volume as part of this treatise.
The idea on which the 15 chapters of the book are based is one of information effectiveness and not of encyclopedic coverage. This is simply because the main target of this textbook is represented by young people under training in surgical specialties, who prepare their final exam.
I can say that in this extremely precise volume of authority, the authors have achieved several firsts, under the careful, effective and professional guidance of Prof. Lucan:
· they have clearly evidenced the importance of clinical examination and adequate history taking, which facilitates correct, rapid and not least, cost-effective diagnosis;
· the illustrations of this volume belong to the information archive of the Clinical Institute of Urology and Renal Transplantation Cluj-Napoca and certify the large amount of work undertaken by the team of this institution, as well as the impressive variety of cases. This happens when the institution concerned has a remarkable addressability, which demonstrates the quality of medical care;
· the clear modality of approach, structuring and presentation of specific subjects facilitates the rapid understanding of the subject and is ideal for those who either train to join this specialty or need rapid information but do not have the time or the disposition to have recourse to an encyclopedic textbooks.
In brief, the basic starting idea which is confirmed by the introductory word of the editor is that of effectiveness from the first to the last page.
Certainly, for some of those who will read this book, the concise text may raise question marks (we must admit that the presentation of the essentials in a concise and clear manner is in fact much more difficult than the development of the same topic in an extensive editorial space).
The list of references is extensive and adapted to the possibility of supplementing the information provided by this volume, these references being highly updated.
Of the 15 chapters of the book, I was particularly interested in the chapter of Semiology of the Urinary System, Specific Urinary Tract Infections, and Laparoscopy in Urology. I was interested in these because they seemed to me the most accomplished of all chapters.
Obviously, the chapter approaching chronic renal failure concentrates and presents an impressive specific experience in renal transplantation, that of a center that can be considered one of the most effective centers of transplantation in Eastern Europe.
There are many treatises of urology available. Some of them have a remarkable quality due to both the quality of the authors and the expertise in treating and analyzing the specific subjects.
The majority of these, in particular the most recent ones, contain very many chapters of paraclinical investigations that use the most modern diagnostic systems. The truth is that the urinary tract, through the specificity of its anatomical design, can be extremely accurately diagnosed. However, very few of these treatises (except the Campbell Treatise) evidence an extremely important fact: clinical diagnosis and history taking will always be the foundation of any diagnosis in urology.
Urology is an important specialty, which is different from the other surgical specialties. This statement is based not only on the fact that there is almost no important personality in the world who did not, does not or will not need the advice of a urologist. It also refers to the place that urology occupies in terms of the incidence of urologic diseases at a general level.
Considering this, the number of specialists in urology should be continuously renewed by serious enthusiastic young people, capable of passing on the scientific message of their predecessors. One of the basic elements that this young generation needs in order to come as close as possible to the achievements of their predecessors is to be able to receive the necessary information in a clear, correct and attractive manner. These requirements are largely met by the volume published by Prof. Dr. M. Lucan.
Any medical textbook resulting from the contribution of several authors represents a potential danger not only to the scientific unity of the book, but also to the unity of collaboration between the various authors and of the way in which the topics are approached up to their final development.
Finally, the book editor has to make uniform the subjects written by each author in order to maintain the idea of a consistent scientific message which was intended from the beginning.
There still is an obvious feeling that was extremely well expressed by Sir Dn. Innes Williams: “The majority of authors of chapters would be much happier to write these chapters for their books than for another editor”.
This is not the case of this book. Although this volume is internationally multi-authored, the uniformity of the chapters, which all express brevity and effectiveness, is clearly seen, without altering the specific personality of the 16 authors from all the different European centers.
The concentration of so many new concepts in one volume of urology demonstrates high editorial maturity. The errors or omissions that may frequently occur are remarkably reduced in this volume.
To conclude, Benjamin Jowete, a professor of literature at Oxford, said: “If you can condense a large book into a small book and keep the original meaning and content of information, you will not have lived in vain”.
